2021 Madison Chapter - Annual Golf Outing

• A. Foursome – $500.00
  Please include the names for three additional people to complete your foursome.
  
  Registration Includes: 18 holes of golf w/cart, 2 drink tickets, lunch, one raffle ticket for prizes and prizes for 1st Place, 2nd Place and one randomly drawn team, plus 10 flag event prizes.
  
  Raffle tickets will be drawn for prizes during golf and available for pick up after your round. Check to see if you are a winner. Winners of places and events will be handed out if staying after for patio mingling, or notified after the event.
• B. Individual Golfer – $125.00
